Getting Started with LifeNet™

• 1. Choose the plan that is best suited for you from the descriptions on the Plan page of the      website.
• 2. Go to the Shopping Cart and purchase your card.
       a. You will receive a username and password within 24 hours via email.
• 3. Go to the Member page on the LifeNet™ Card website.
       a. Using the confidential username and password, log on to the Member page.
       b. At this time, if you choose, you may change your username and password to a personal          selection.
• 4. WELCOME, you are on your way.
       a. At the top of each form, we show you the information to gather before you begin.
       b. For your ease, we suggest you fill in the forms in the order they are listed.
       c. As you complete each form, click on Submit to save your information. You can go back          to the form, even after you have moved on to the next forms, by just opening the window          back up again.

• 5. We estimate that you will be able to complete all of the forms for all three Plans to your      satisfaction in less than 2 hours.
       a. Upon completion, go to the Data Verification Form and follow the instructions.
       b. When you click the final Submit button on the Data Verification form, your information is          downloaded to our secure storage files.
• 6. Your information will be immediately loaded onto your new personal ID Card.
• 7. At the time your card is mailed to you via U.S. Mail, you will receive a confirmation email      from LifeNet™.
• 8. Insert your new personal LifeNet™ card into the USB port of your computer and verify the      accuracy.
