LifeNet™ Plans

You have the opportunity to choose two different ways to input your information:

1) Enter the information online at the LifeNet™ website www.lifenetcard.com.

2) Receive the forms via the U.S. Postal Service, fill in the necessary information and return the forms to LifeNet™ to have the information entered for you

Depending on the plan you choose, you will be given a list of forms to complete. As you move from one form to another, we will not ask you to fill in any information that you have already filled in on a previous form. You will find that it has already been transferred to the proper location. More importantly, you have control over the level of information stored on your LifeNet Card™. Depending on the amount of data you would like to store, an initial review of the forms and taking the time to gather it all together before you begin will ease the input process.

After you have submitted and verified the accuracy of your information, it will be downloaded onto your LifeNet Card. Your personalized card will then be shipped directly to you ready for you immediate use.
